All-on-4 vs All-on-6 in Turkey: How Dentists Choose
Why more implants are not automatically better—and the clinical factors that guide a full-arch treatment plan.

Das Wichtigste auf einen Blick
Neither All-on-4 nor All-on-6 is automatically best. A recent systematic review found broadly similar clinical outcomes but substantial variation between studies, so implant count alone should not drive the decision. The treating team should justify the design using three-dimensional imaging, bone and anatomy, implant distribution, bite, prosthetic space, maintenance and patient health.
Wichtigste Erkenntnisse
- All-on-4 uses four implants per treated arch; All-on-6 uses six.
- More implants may offer different force distribution or redundancy, but require suitable anatomy and are not automatically safer.
- The prosthesis design, implant position, hygiene access and maintenance plan matter alongside implant count.
- “Teeth in a day” usually refers to a provisional restoration, not completed biological healing.
- Ask what happens to the restoration if one implant cannot be used.
All-on-4 and All-on-6 describe fixed full-arch restorations supported by four or six implants. The number is easy to market; the harder and more important questions concern bone, implant position, prosthetic design, hygiene access, bite forces, surgical risk and what happens if one component develops a problem.
What do All-on-4 and All-on-6 mean?
They are treatment concepts in which a fixed full-arch dental prosthesis is supported by four or six implants in one jaw.
The visible result is not four or six individual teeth. It is a connected full-arch prosthesis supported by implants placed in planned positions. Implant angulation, available bone and the extension of the prosthesis all affect the design.
The label also does not define every material, surgical step or temporary stage. Ask whether the quote refers to the provisional bridge, the final bridge or both.
Is All-on-6 better because it uses more implants?
Not automatically. Additional implants can change support and force distribution, but benefit depends on anatomy, placement and the prosthetic plan.
A 2026 systematic review comparing All-on-4 and All-on-6 reported broadly similar outcomes and advised caution because the included studies were heterogeneous. That means the evidence does not support choosing solely from a marketing claim that one number is universally superior.
Six implants may not be feasible or useful when bone volume, sinus position, nerve location or restorative space limits placement. Four well-planned implants may be appropriate in one case, while another person may benefit from a different number or an alternative design.
Which factors should determine the design?
The design should reflect anatomy, bone, health, bite, implant distribution, the planned prosthesis and the ability to clean it.
Three-dimensional imaging is commonly important for planning full-arch implant positions. The final plan should also explain whether extractions, grafting or gum treatment are indicated and how those steps affect the schedule.
| Factor | Why it matters | Question to ask |
|---|---|---|
| Bone and anatomy | Limits safe implant size and position | What does my scan show at each proposed site? |
| Implant distribution | Affects support across the arch | Why were these positions selected? |
| Bite and habits | Influence forces on implants and prosthesis | How are grinding or strong bite forces managed? |
| Prosthetic space | Affects material thickness and cleansability | What final bridge design is planned? |
| Health and hygiene | Affect healing and long-term tissue stability | What risk control is required before treatment? |
| Contingency | Plans for an implant or component problem | Can the restoration be modified or repaired? |
What does immediate loading or teeth in a day mean?
It usually means a provisional bridge may be attached soon after implant placement when clinical stability and other criteria are met.
Immediate loading is not suitable in every case and does not mean the implants have already completed healing. The provisional restoration may have dietary and bite restrictions while integration progresses.
Ask what conditions must be met on the day, what alternative is planned if stability is insufficient and when the final prosthesis will be made.
How do cleaning and maintenance differ from natural teeth?
A full-arch bridge needs daily cleaning around and beneath the prosthesis plus professional maintenance tailored to the design.
The team should demonstrate the cleaning tools and access points before you leave. Ongoing reviews look at tissues, plaque control, bite, screws, wear and any change in fit.
Poor plaque control, smoking, a history of periodontitis and uncontrolled diabetes are among the factors that require careful risk discussion and follow-up around implants.
What should a full-arch quote specify?
It should specify the clinical stages, implant system, provisional and final prostheses, materials, extra procedures, return visits and aftercare.
Health Talia does not publish a fixed full-arch list price. A free x-ray review starts the assessment, and the exact quote follows the individual plan. Potential savings of up to 30% should be compared using the same clinical scope and material level.
- Number and brand or system of implants.
- Provisional restoration and final restoration as separate items.
- Final prosthesis material and framework.
- Extractions, grafting or other preparatory treatment.
- Expected visits, healing checkpoints and hotel or transfer inclusions.
- Written aftercare, maintenance and guarantee conditions.
Häufig gestellte Fragen
Kurze Antworten auf die häufigsten Fragen internationaler Patienten.
Is All-on-6 stronger than All-on-4?
Not as a universal rule. More implants can change support and force distribution, but anatomy, implant position, prosthesis design, bite and maintenance determine whether that is beneficial in an individual case.
Can everyone have teeth in a day?
No. A provisional bridge may be fitted quickly only when the clinical conditions support immediate loading. The dentist needs a fallback plan if those conditions are not met.
Do All-on-4 implants need cleaning?
Yes. The tissues and spaces around and beneath the bridge require daily cleaning and professional maintenance. Fixed does not mean maintenance-free.
Can one failed implant affect the whole bridge?
It can affect the plan, depending on implant position and bridge design. Ask before treatment how the team would manage an implant that cannot be loaded or later develops a problem.
How is All-on-4 suitability checked remotely?
A panoramic x-ray and health information can start a preliminary review, but final planning generally needs clinical examination and may require three-dimensional imaging.
